1/7/2023 EnchantaVaughn Approved 1. Behavior: Client was present and very relaxed when QMHP arrived. Client greeted the QMHP in a friendly manner. Client was happy to see her counselor and thanked her for coming to see her. Mom appeared to be in a joyful mood and grateful to get a break from the kids since both counselors were there to support them. Mom explained that she wanted client to get the most out of in-home services and will continue to encourage her to participate in each and every session. Client expressed cheerfulness and positive affirmations when given the opportunity to speak. Client shared that she was having a great day and getting along with everyone in the home. Client seemed to be in good spirits. Client presented with a bright affect. Client was very attentive and cooperative in the session. 2. Behavior: Client waited for instructions, displaying polite and calm manners during the second half of the session. Client was supported as she focused on the therapeutic activity. Client was in a happy mood and expressing healthy emotions toward the activity provided by QMHP. Client was given a thumbs up for showing positive emotions and exhibiting empathy toward in-home staff. Client displays good behaviors during the session. Client displays age appropriate skills during one on one; avoiding conflict, staying on task and focusing on herself. As Client was provided with a calm environment and reassurance that she would be cared for, her level of agitation has decreased. Client shared that she is learning to develop the skills needed to achieve her goals on a daily basis and is proud at how far she has come. 1. Intervention: QMHP explored positive behavior options with client to support her willingness to make good decisions in the home. QMHP engaged in conversation with client about positive decisions and her plan to continue building relationships with her mom and sibling. QMHP inquired about evening routine and after school activities client is currently involved in. QMHP assisted client in monitoring her own behaviors in efforts to promote exercising self-control in challenging areas. QMHP processed client’s thoughts and feelings during self-reflection. QMHP supported client in approaching her family with new and innovative ideas. QMHP continued to establish a positive supportive relationship with Client. QMHP will review and give feedback regarding ADHD and its causes and effect to well-being at home, as well as, his performance in the program. 2. Intervention: QMHP reiterated the expectations and rules for the session. QMHP applauded client for wanting to work on changing her behavior so that she leads by example and takes responsibility for her actions. QMHP reviewed client after school schedule with her and supported as she prepped for the evening work she had. QMHP commended client on utilizing creative strategies to increase her understanding of feelings and emotions as it relates to her mental illness. QMHP encouraged client to continue displaying positive behaviors. QMHP praised client for her cooperation. QMHP praised and recognized client for her hard work, efforts, participation, and working towards improvement. QMHP praised client for her commitment in trying to meet her objectives daily. 1. Response: Client responds well to praise and support. Client implemented good communication skills by listening attentively. Client verbalized good thoughts and understanding. Client developed better communication skills by expressing herself in a calm manner. Client displayed management of individualized behaviors. Client has not overreacted with anger to minor frustrations or irritants. Client responded in a pleasant manner acknowledging the positive reinforcement that the QMHP provided. Client was cooperative and compliant. Client displayed appropriate social behavior. Client was sociable and friendly while playing with her little sister. Client responds well to the positive reinforcement provided by QMHP and is seen increasing self-esteem and confidence. Client have adapted and is observed modeling appropriate behavior at this time. 2. Response: Client expressed positive self-talk, modeled positive behavior, and good communication skills during role play activity. Client is making progress with expressing her feelings and showing others respect and courtesy by talking with appropriate tone, initiating conversation, and waiting for her turn to talk. Client responded well to feedback. Client communicated a positive attitude when reviewing problem solving and self-control strategies with her support system. Client expressed excitement, demonstrating mindfulness and seemed to be motivated to do the right thing. Client presents a positive attitude when given the opportunity to earn a reward. Client was happy and in a good spirit throughout the entire session. Client responded well to QMHP and appeared thankful for the extra help and support.
